Abstract

This submission focuses on the application of ACM to address the dynamic requirements of healthcare practitioners, offering a framework that transcends traditional decision support and knowledge management systems, moving towards a patient-centric approach that emphasizes teamwork and collaboration as key aspects of the healthcare process. Illustrated are how patients within such a collaborative model follow an Integrated Care Pathway (ICP) involving multi-professional care team members providing different healthcare services at distributed sites. In contrast with current Hospital Information Systems (HIS) used at different healthcare organizations, that were originally designed to support the traditional disease-centered delivery model, ACM provides a foundation for effective mechanism to support the interaction and collaboration among the care team members, as well as notably the management of all actual and potential interactions between the various complex care pathways which are followed by each individual patient.
